Our practice has partnered with Nsight Health to help improve care between appointments.
Nsight Health provides remote patient monitoring and care coordination services that allow our team to stay connected with patients at home, not just during office visits.
Through daily vital monitoring, monthly check-in calls, and 24/7 clinical availability, patients receive ongoing care support while your provider stays informed.
How the Program Works
If your provider determines you are eligible, you may be invited to participate in the Nsight Health program.
Once enrolled:
- You may receive a medical device to measure vital signs from home
- You’ll be asked to take ongoing readings
- Nsight reviews your data for any changes or concerns
- You receive a brief monthly call from an Nsight Care Coordinator
- If something appears concerning, Nsight communicates back to your provider
- Your care remains directed by your physician at all times
Most readings take under a minute, and monthly calls usually last about 15 minutes
Enrollment Outreach
If your provider identifies you as eligible and recommends you for the Nsight program(s), a member of the Nsight Health Care Team may contact you directly to explain the program and help you get enrolled.
During this call, they will:
- Confirm your interest
- Review program details
- Answer your questions
- Coordinate delivery of any devices (if applicable)
- Confirm contact information
This call helps make enrollment simple and ensures you receive the support you need from the start.
What Patients Receive
Nsight Health supports patients with:
- Daily health monitoring (when applicable)
- Monthly nurse check-in calls
- Ongoing engagement and care coordination
- Medication support and education
- Assistance with appointments and care needs
- Device setup and troubleshooting
- Support with refills and care reminders
- Communication with your provider when needed
You can think of it as having an extended care team working alongside your doctor.
Clinical Support You Can Rely On
Nsight Health offers 24/7/365 clinical support for enrolled patients.
This includes:
- Access to trained care staff
- Support for health questions and concerns
- Escalation to your clinic when medically necessary
- Patient outreach and follow-up
Monitoring Devices (If Eligible)
If a device is part of your care plan, Nsight Health assists with:
- Shipping and setup
- Device education
- Replacement and maintenance
- Ongoing technical support
Your provider will determine whether a device is appropriate for you.
